About the Role
We are seeking an experienced NDIS Coordinator of Supports to join a community-focused organisation based in Port Macquarie. This is an urgent requirement with an immediate start, offering an initial contract of 2–6 months with the potential for extension. You will play a key role in supporting NDIS participants to implement and maximise their plans, coordinate services, and achieve positive outcomes aligned with their goals. Working across a varied caseload, you will collaborate with participants, families, service providers, and stakeholders to ensure supports are effectively coordinated and delivered in accordance with NDIS requirements.
 
Duties
  • Support participants to understand and implement their NDIS plans and funding
  • Coordinate services and connect participants with suitable providers and community supports
  • Monitor participant progress and outcomes against established goals
  • Assist participants in preparing for plan reviews and reassessments
  • Build participant capacity to exercise choice, control, and independence
  • Attend case conferences, reviews, and stakeholder meetings as required
 
Skills and Experience
  • Relevant qualification in Disability, Community Services, Social Work or Health
  • Demonstrated experience in NDIS Support Coordination, Coordination of Supports, Case Management, or a similar role
  • Strong understanding of NDIS processes and compliance requirements
  • Excellent communication, stakeholder engagement, and relationship-building skills
  • Experience working with participants with complex support needs highly regarded
