Role Overview
The Clinical Care Partner is responsible for coordinating and managing high‑quality, person‑centred aged care services for clients receiving Home Care Packages and related supports. This role combines clinical expertise with care coordination, ensuring services are safe, compliant, and responsive to individual needs.
The position suits an experienced Registered Nurse (RN) or Enrolled Nurse (EN) with strong knowledge of community‑based aged care, excellent communication skills, and a commitment to quality outcomes.
The Clinical Care Partner works collaboratively with clients, families, support workers, allied health professionals and internal teams to deliver coordinated, compliant and compassionate care.
Key Responsibilities
Care Coordination & Clinical Oversight
- Coordinate Home Care Package services in line with individual care plans and funding guidelines
- Conduct comprehensive client assessments, reviews and reassessments
- Identify clinical risks and escalate appropriately
- Support medication management and clinical monitoring within scope of practice
- Ensure continuity and quality of care delivery
- Build trusted relationships with clients and families
- Support clients to achieve their goals and remain independent at home
- Respond to client enquiries, concerns and feedback in a timely and professional manner
- Ensure care delivery is culturally safe, inclusive and person-centred
- Provide guidance and clinical support to care and support staff
- Communicate client needs, risks and changes clearly to the care team
- Contribute to staff onboarding, education and ongoing capability development
- Encourage a positive, values‑driven team culture
- Ensure compliance with Aged Care Quality Standards and organisational policies
- Maintain accurate and timely care documentation and progress notes
- Participate in incident management, audits, and continuous improvement activities
- Support accreditation and regulatory requirements
- Liaise with internal scheduling, administration and leadership teams
- Support efficient service delivery and resource use
- Contribute to service improvement initiatives and best‑practice care models
- Current AHPRA registration as a Registered Nurse or Enrolled Nurse
- Minimum 2 years’ experience in aged care or community nursing
- Sound knowledge of Home Care Packages and aged care quality standards
- Strong clinical assessment and care coordination skills
- Excellent communication, time management and organisational abilities
- Current NSW Driver’s Licence and reliable vehicle
- Experience in community‑based aged care services
- Knowledge of clinical governance and risk management
- Experience using care management or client record systems
- National Police Check
- NDIS Worker Screening Check (where applicable)
- Working with Children Check
- Current First Aid and CPR
