We are seeking experienced Care Coordinators to join a healthcare organisation on a short‑term 6‑week contract based in Bowen Hills. This role is suited to candidates who are comfortable managing a portfolio of clients, coordinating services, and supporting care delivery in a fast‑paced, service‑driven environment.

Key Responsibilities

• Act as the primary point of contact for a portfolio of healthcare clients
• Coordinate and schedule care and support services to meet individual client needs
• Manage inbound and outbound calls, providing clear and empathetic support
• Maintain accurate case notes and client records across multiple systems
• Support onboarding of new clients and provide ongoing coordination support
• Monitor service delivery and escalate issues as required
• Work closely with internal teams and service providers to ensure continuity of care

• Experience in care coordination, case management, or account management roles
• Background in healthcare, aged care, disability, or community services preferred
• Comfortable working in a call‑centre style, high‑volume environment
• Strong customer service and communication skills
• Ability to manage sensitive conversations with professionalism and empathy
• Strong organisational skills with the ability to manage competing priorities
• Confident using multiple systems and technology platforms
