Assessment & Care Planning Support
Contract: ASAP Start Until Christmas (Potential Temp-to-Perm)
Rate:$43.78 per hour + Super
Location: Remote or Brisbane-Based

We are seeking experienced and motivated professionals to join our healthcare support team in Assessment and Care Planning Support roles. These positions offer an immediate start through to Christmas, with the potential for extension or transition to permanent employment.
 
Key Responsibilities

  • Conduct and support assessments to identify client needs, strengths, and service requirements.
  • Assist in developing, reviewing, and updating care plans to ensure alignment with clinical recommendations and client goals.
  • Collaborate with multidisciplinary teams to coordinate care delivery and support quality outcomes.
  • Document and maintain accurate client information using electronic health record (EHR) systems in accordance with privacy and compliance standards.
  • Provide administrative and coordination support to facilitate timely assessments and service planning.
  • Monitor client progress and escalate issues or risks to relevant team members as required.
  • Contribute to continuous improvement initiatives within the assessment and planning process.
 
Requirements
  • Background in healthcare, social services, or a related field.
  • Strong assessment, analytical, and communication skills.
  • Proven ability to work independently and manage workload in a remote environment.
  • Proficiency with technology and EHR systems.

Desirable
  • Previous experience in care assessment, coordination, or planning.
  • Knowledge of healthcare regulations, compliance, and quality standards.
  • Qualifications in Nursing, Social Work, or Allied Health.
  • Additional language skills or experience in diverse healthcare settings.
 
Additional Information
  • Remote work available Australia-wide.
  • Hardware provided for Brisbane-based employees; remote candidates must have a suitable home workspace.
  • Contract period: ASAP start until Christmas, with potential to extend or transition to a permanent role.
