Chandler Macleod are proud to work with a Federal Government agency designed to enhance the lives of many Australians across the country. They currently have a number of APS6 Planner roles based in Midland to commence ASAP for 12 months.

Pay: $51.50 per hour + super

As a Planner, you will liaise with participants, families and carers, to identify what supports are required to achieve a participant's goals.

The duties will consist of but not limited to:

  • Conducting initial needs assessments of participants
  • Determining functional and personal limitations to develop long term goals
  • Implementing plans to make progress with a person's goals and aspirations
  • Engaging with community groups and build awareness to improve opportunities for people with disabilities
  • Liaising with health and other support providers to gather information to facilitate plan development.
  • Undertaking plan reviews with participants
  • Interpreting and adhering to disability legislation

The ideal candidate will possess:

  • Competency in applying assessment tools to identify support needs
  • Experience with supporting people with a disability
  • Strong and adaptable communication and interpersonal skills
  • High capability in relationship building
  • Ability to manage confidential and sensitive information
  • Ability analyse complex information and make decisions
  • Ability to adapt quickly to a changing environment
  • It is highly desirable that applicants have an understanding or lived experience of disability
  • It is highly desirable that applicants have relevant qualifications in human services, allied health or disabilities
