About the role:

  • Liaising with participants, their families, carers and support partners to identify what supports are required to achieve the participant's goals
  • Gathering information from participants and their families - either face to face or in person, to enable planning to meet their needs and requirements
  • Calculating of funding for individual plans and assisting with the allocation of the funding to suit the needs of the participant
  • Determining functional and personal limitations to develop long term goals
  • Implementing plans to make progress with a person's goals and aspirations
  • Undertaking plan reviews with participants
  • Interpreting and adhering to disability legislation

About you:

  • Experience with supporting people with a disability
  • Strong and adaptable communication and interpersonal skills
  • High capability in relationship building
  • Ability to manage confidential and sensitive information
  • Ability to adapt quickly to a changing environment
  • It is highly desirable that applicants have an understanding or lived experience of disability
  • It is highly desirable that applicants have relevant qualifications in human services, allied health or disabilities
  • You will have a driving license and your own vehicle, as you may be required to attend remote or regional areas
