The Company

Our client is a community controlled health service, which aims to deliver primary health care services, advocacy and social and emotional support to the Indigenous community.

The Role

The Disability Services manager will lead and mentor the Disability Support Team to deliver professional and effective support services, as well as working directly with clients to provide disability support coordination that will enrich their lives and enable them to continue to live in their own homes and be successful members of the community. You will be responsible for: -

  • Support coordination for NDIS participants
  • Conducting needs based assessments with participants, based on their NDIS goals
  • Overseeing onboarding of new participants
  • Completion of NDIS administration tasks, including billing and compliance
  • Lead, mentor and coach the Disability Support Team to ensure provision of effective support services
  • Oversee allocation of resources and rostering
  • Performance management

The Successful Applicant

To be successful in this role you will have experience working within a similar role within the Disability, Health or Aged Care sector, along with: -

  • Diploma in Community Services (Disability), or relevant experience
  • National Police Check, Working with Children Check and current NSW drivers license
  • Qualifications in Mental Health will be highly regarded
  • Sound understanding of relevant legislation and NDIS procedural guidelines
  • Demonstrated skill in establishing empowering and supportive partnerships with participants
  • Demonstrated respect and understanding of the Health and Social issues impacting local Indigenous people
