Our client, a large government department, requires an APS6 Clinical Care Coordinator to join their team on a 12-month contract, with the potential for extension. This role will be based in Surry Hills and pay $55-65 per hour + Superannuation.

The AP6 Clinical Care Coordinator will work with clients and internal and external stakeholders to enhance the agency's ability to identify and respond to the needs of vulnerable and at risk community members and/or their families.

The position takes a strengths-based, recovery approach to supporting vulnerable and/or at risk persons to enhance wellbeing and participation in their community through community engagement, collaborative recovery work, case coordination, complex case management and linkage with group programs and external services.

The role will involve providing phone or in person assessment, support and linkage to internal and external services to clients presenting with immediate needs.

Key Responsibilities:

  • Direct provision of high quality, evidence informed clinical services including intake, assessment and referral, complex case management, and case coordination to a caseload of clients with identified complex and/or multiple needs
  • Maintain clinical records and analyse data from client records, and prepare reports
  • Work with stakeholders to influence a recovery approach to service delivery within a stepped care model for vulnerable or at risk persons
  • Participate in community development and service promotion activities
  • Participate in and contribute to ongoing learning and development activities and the maintenance of professional status
  • Participate in evaluation and research activities, contribute to project management activities that implement strategic direction and quality improvement recommendations, and provide feedback for ongoing development of clinical policies and procedures

Key Requirements:

  • Tertiary education in one of the following areas - Psychology, Occupational Therapy, Accredited Mental Health Social Worker, or be a Nurse with mental health experience
    • Qualified Psychologists, Occupational Therapists and Nurses must have registration with APHRA
    • Mental health accredited Social Workers must be registered with AASW
  • Relevant experience within a similar role will be highly desirable
  • Experience in dealing with persons with mental illness and/or vulnerabilities will be beneficial
  • Working with Children's Check
